WELCOME TO PARK SLOPE ORAL SURGERY!

Our staff welcomes the opportunity to provide you with an enjoyable experience while you are here, especially on your first visit.

Your initial appointment with one of our surgeons will include a consultation to explain your diagnosis and treatment options. We can also schedule our surgeons to perform surgery on the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. After all, we want to get you back to your normal daily activities as soon as possible.

REQUIRED FIRST-VISIT FORM

The purpose of the first-visit form is to help us provide you with the highest level of care. If you have any questions, please ask for assistance.

It would help you save time on your first visit by filling out the form below and bringing it to your appointment. Make sure to bring all insurance information, a photo ID, and be sure to alert the receptionist if you have any medical conditions that may be of concern before surgery.

MEDICAL HISTORY

Those taking medications should bring the name and dosage of the drug(s) to their appointment. Having all of this information shortens your wait time.

INHALATION SEDATION

If you prefer nitrous oxide (laughing gas) for sedation during the surgery, limit your oral intake to a light meal up to two hours before the procedure.

MINORS

A parent or guardian must accompany any patient under the age of 18.

REFERRAL SLIP AND X-RAYS

Patients referred by a dentist should bring the referral slip and x-rays to their appointment. In some cases, X-rays may be inadequate and will be retaken in our office. You do not have to be referred to our office by another doctor or dentist. We accept patients directly, so feel free to call to set up an appointment.

If you do have x-rays: If your dentist or physician has taken x-rays, you may request that they forward them to our office email (parkslopeos@gmail.com). Even if you have recent routine dental films, we often need to take a panoramic X-ray to more carefully evaluate your teeth and the supporting jawbone.
